all bid and performance bonds, please provide: 1. Fully completed application. 2. Latest business and personal
tax returns - first five pages only. 3. Latest business year end financial statement prepared by a CPA, if available,
for requests in excess of $250,000 4. Personal balance sheet. 5. Details on project to include overall bid amount,
time to complete, delay damages - if any and term of warranty when completed.

All School Bus Contractor Applicants are Required to Provide the Following Items in Addition to the Application:
- Personal Tax Returns, first 10 pages (on all partners)
- Corporate Tax Returns, first 10 pages

End Corporate Financials
- Latest 3 Months Bank Statements, Corporate & Personal
- Certificate of Liability
